Our Patient Care Model

Initial Assessment:

The care team has an initial visit at the patient’s location. In this meeting, the care team will learn about the patient’s health issues and history, goals, and the level of support available from family and caregivers.

Based on this information, a personal care plan is developed that includes:

• Medication Management
• Symptom Management
• Disease Management
• Coordination with other medical specialists
• Communication with family and caregivers
• Coordination of in-place care and support services
• 24/7 support, including telemedicine services with videoconferencing and real-time monitoring and interpretation of physiological data

Primary Care:

Tulane Doctors – Connected Care is their patient’s first point of care. The medical professionals coordinate all preventive and chronic care, working with medical specialists based on patient needs. All care is provided in the comfort of the patient’s current residence or facility.

Hospital Care:

A team of hospitalists work with the entire hospital-based care team to manage the patient’s hospital stay, and reduce readmissions.
